The Department encourages our faculty to organize field trips that will enhance our students' learning. In order to provide some guidance in the rights and responsibilities of the participants we have consulted with the Office of Environment, Heath and Safety in 2007-08 to create this
DRAMATIC ARTS URBAN and LOCAL FIELD TRIP INFORMED CONSENT, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT

Please note that this agreement must be completed in full, signed, dated, and witnessed and paragraphs initialled before the participant is allowed to participate in the activity. These forms AND a list of participants must be left with the Departmental Chair or his/her representative before the field trip occurs.
If you require mandatory participation in the fieldtrip then it should be included in the course outline you distribute to students. Otherwise you should include a statement that fieldtrips will be scheduled and that mandatory participation is required, exceptions being possible under the regular terms. Please consult the Chair if you have a question about this.
The form provided above does not provide for rural or long distance field trips. Please contact the Chair.
